Our Services

The Interdisciplinary Team.

At Lindenwood Hospice and Palliative Care, an interdisciplinary team of medical professionals and support staff works together to provide compassionate, high-quality care to patients facing a life-limiting illness. This team includes the hospice medical director, registered nurses (RNs), licensed vocational nurses (LVNs), certified nurse aides (CNAs), medical social workers, chaplains, and trained hospice volunteers.

The team works together to create a personalized plan of care for each patient, focused on addressing the patient's physical, emotional, and spiritual needs. The team also encourages family participation in the care process and provides support to both the patient and their loved ones during this difficult time.

Personal Care.

Certified nurse aides (CNAs) provide personal care services to hospice patients, assisting with tasks such as bathing, grooming, changing bed linens, and tidying up the patient's area. Personal care services help to promote comfort and dignity for patients as they approach the end of their lives.

Volunteer Support.

Trained hospice volunteers provide caregiver respite, companionship, and can assist with day-to-day tasks. Remote communication opportunities are also available. Volunteers are an important part of the hospice team, providing much-needed support to patients and families during this difficult time.

Medical Services.

Lindenwood Hospice and Palliative Care provides a range of medical services to patients with a certified terminal diagnosis and prognosis. The hospice medical director oversees all medical care provided to each patient, working alongside the patient's primary physician to manage pain and other debilitating symptoms.

Registered nurses (RNs) and licensed vocational nurses (LVNs) provide care in accordance with the plan of treatment, working with the interdisciplinary team to ensure that symptoms are addressed and the patient is as comfortable as possible.


Medical Social Services.

A licensed medical social worker is available to provide emotional support to patients and their families during this difficult time. The social worker works to alleviate stress related to the illness, provides support, and assists in long-term planning when appropriate. The medical social worker is an essential member of the interdisciplinary team, working closely with other team members to ensure that the patient's physical, emotional, and spiritual needs are addressed.

Skilled Nursing

Skilled nursing is an essential part of hospice care, providing expert medical care to patients with complex health needs. At Lindenwood Hospice and Palliative Care, skilled nursing services are provided by registered nurses (RNs) and licensed vocational nurses (LVNs) who work closely with the interdisciplinary team to provide personalized care to each patient.

Spiritual Care

At Lindenwood Hospice and Palliative Care, spiritual care is provided to patients and their families based on their preferences. The hospice chaplain or clergy services are available to offer prayer and pastoral counseling to patients and their loved ones, whether in person or via telehealth visit. Spiritual care is an important part of hospice care, providing comfort and support to patients and families as they navigate end-of-life issues.

Bereavement Support.

Support is provided to the family of the patient over a period of 12 months after the patient's passing. Once a year, a celebration of life and remembrance is held to honor loved ones. The bereavement support program provides emotional support and counseling to help family members navigate the grieving process.

How to refer a patient to Lindenwood Hospice & Palliative Care.

Referring a patient to Lindenwood Hospice and Palliative Care is simple. To begin hospice services, a consultation for certification of the terminal condition and prognosis by the hospice medical director is required. Once certified, verification of hospice benefit is obtained, and an initial visit is conducted by the RN and interdisciplinary team. The visits are scheduled according to the physician's plan of care and the patient's needs. To refer a patient,

simply call us at 1-800-395-9353, and our hospice RN will conduct an initial assessment within 24 hours of approval. We will work closely with the patient's healthcare providers to ensure a smooth transition to hospice care.
